I do my best to help as many clients as possible. Completion of an application is not a guarantee of service. I am passionate and dedicated to my clients and will only work with those who are equally dedicated. Please understand I am not Wonder Woman, despite the fact we have never been seen together, I cannot solve all the issues in one session. Progress/success is dependent on your dedication and practice outside of sessions.

Before you proceed:

1. You will be required to provide proof of rabies. A tag is does not provide sufficient information. Please obtain the rabies certificate from your vet before proceeding.

2. For security purposes, this application will not save your progress until you click "submit". Please allow yourself at least 15min to complete this application and do not leave or close the page for any length of time until finished, unless you want to risk restarting.

3. Please be mindful! This application will ask a lot of questions you may feel are irrelevant however I promise they are important. Rushing through this application and providing minimal information will hinder my ability to help you and ultimately lead to denial of service.
