Therapy Dog Prep Application

This application is for the Group Therapy Dog Prep Classes. This is a 6 week Course. Completion of this application does not guarantee enrollment in an upcoming class. This application is one of two steps to secure enrollment in an upcoming class. The next step is to attend an upcoming Meet & Greet. Once the application is reviewed, you will be contacted and invited to join the THCC community on the Spaces app. This platform is an extension of my website. There you will be able to RSVP to an upcoming Meet & Greet.

Attendance to this Meet & Greet is mandatory to secure enrollment in an upcoming class. Please contact me with questions regarding this process before completing this form.

Before you proceed:

• You will be required to provide proof of rabies. A tag is does not provide sufficient information. Please obtain the rabies certificate from your vet before proceeding.

  
  

Meet & Greet

During the Meet & Greet, teams will be evaluated to determine if this class will be beneficial. Teams are evaluated on communication and handling skills and the dog's temperament. Those who are invited to join will make payment (card or cash only) and be enrolled. Space is limited so spots are offered based on date of receipt of application.
